Looking to mail a check or cash to the FSHD Society on behalf of a participant or team? Checks can be mailed to: FSHD Society * PO Box 411617 * Boston, MA 02241-1617* Include the FORM with the name of the participant, their team (if applicable), and the event location in order to ensure proper credit to their fundraising account!
